Please read the information below. Feel free to call me with any questions.
Physical Examination:
A physical exam by a licensed healthcare provider is required upon admittance to Hatikvah. A physical exam performed within 12 months of school entrance is acceptable.
Immunizations:
The immunizations listed below are required by the State of New Jersey for school admission. Proper documentation of these immunizations must be provided to the school nurse upon enrollment.
- Dtap/DPT -4 doses (4th dose must be given on/after 4th birthday or a total of 5 doses)
- Polio-3 doses (3rd dose must be given on/after 4th birthday or a total of 4 doses)
- MMR-2 doses (1st must be given after 1st birthday)
- Hepatitis B-3 doses
- Varicella/chickenpox-1 dose(given after 1st birthday) 2nd dose recommended.
- Influenza – Recommend 1 dose annually
- Tuberculosis Testing – As per NJ State Department of Health Guidelines
Medication at School:
Medication should be given at home whenever possible. If a medication (prescription or over-the-counter) must be given in school, the following consent form must be completed by both parent/guardian and the student’s licensed healthcare provider.
The medication must be brought to school by the parent/guardian in the original container or prescription bottle.
Screenings:
Several screenings are conducted by the nurse during the school year. A letter is sent home if an abnormality is detected.
- Height, weight, blood pressure – yearly
- Hearing – Grades K-3 & 7
- Vision – Grades K, 2, 4, 6 & 8
- Scoliosis – Grades 5 and 7
Absences and Illness policy:
Please call Stacy Weissman, BSN,RN at 732-640-5850 x 203 if your child will be absent from school. A message may be left on the nurse’s voicemail.

Changes in Medication or Health Status:
Please notify the school nurse if there is a change in your child’s health status, diagnosis, or medication. It is important that the nurse have this information so that she can safely care for your child in the school setting.
Food Allergy Forms:
If your child has a food allergy, the attached forms must be completed and submitted to the school prior to the start of each school year.
- Medication Consent Forms (print 2 – 1 for Benadryl and 1 for epipen)
- Parent Questionaire for Food Allergies
- Severe Allergy IHP
- Consent to Share Information with Staff and Transportation Department
- Consent for Nurse to Exchange Information with Child’s Healthcare Provider
If your child has a diagnosed food allergy, you must supply the school with:
- 2 epipens
- A new bottle of Benadryl
Asthma Action Plan:
If your child has asthma, the attached forms must be completed and submitted to the school prior to the start of each school year.
If your child is prescribed an inhaler, you must supply the nurse with:
- Inhaler
- Spacer
If your child is prescribed nebulizer medication, please supply the nurse with:
- Medication
- Mask and Tubing
There is a nebulizer in the nurse’s office.
